MEMBERSHIP APPLICATION INSTRUCTIONS
COESRA requires an online application process that accepts credit card payments. If your organization only allows for payments by check, it is recommended that you print and send a copy of the completed application to COESRA at P.O. Box 995, Columbus, OH 43216, or FAX to 614-388-5753, to speed the membership implementation process.

Please use a personal email address (Many business email systems have spam filters that will not allow group email). The information provided on the application will be published in the member directory and the answers to the questions will be used to create a guide for the Vendor Reps when marketing their product/service to your company. Membership renewals are sent to the current representative’s email address.
